Request for Infusion Pump for patients (with thalassaemia or other haemoglobinopathies) within NSW: (To be eligible, the patient is required to be a current financial member of the Thalassaemia Society of NSW with highest priority to patients under our umbrella).  Membership/Renewal Application Form
